Change of Details

It is important that we have your correct details at all times, in case we need to contact you regarding your health care, and so that we can continue to receive government funding and pass on subsidized fees to you.  If you are moving house, changing your contact phone number(s), getting married and changing your name, or any other change of details please let us know as soon as you can.  You can complete and submit your changes below or alternatively phone our receptionists on 477-3700.
